Shipped v4.0.0 of markdown-vault-mcp today.

Had a user report their whole vault going dark from a single typo'd note (The files are okay - the tools weren't finding them). Turned out to be two separate crash classes AND a privacy bug (a BOM could silently expose a hidden note). All fixed — one bad file now fails alone, loudly, safely.
